For all membership applications, print off and complete this form and then send to Mrs Di Dimond, Membership Secretary, c/o The Avenue Tennis Club, The Grove, Burnham on Sea, TA8 2PA (07979 227866) or e mail on didimond110@hotmail.co.uk . Membership costs are pro-rata so you only pay for the remaining months of the financial year.

Note for new members JOINING PART WAY THROUGH THE YEAR: you only pay for the remainder of the Tennis year to 31/3/09 – Multiply subscription fee above by number of months remaining and then divide by 12 (not applicable if choosing membership category “1A” – 6 month trial membership).
